The clinic itself.

Where are you located?
One clinic at 500 Terry Francine St, San Francisco CA 94158 — in the Mission Bay neighborhood, two blocks from the UCSF medical campus.

How long have you been open?
We opened in 2018. Going on eight years as an independent, founder-led family practice.

Who founded the clinic?
Dr. John Wang (family medicine) and Dr. Talya Sulami (internal medicine) founded Linden in 2018 after twenty years inside large hospital networks. Both remain practicing partners.

Are you accepting new patients?
Yes — new-patient bookings are open year-round. Most first visits are scheduled within 7–10 days. We'll let you know during onboarding which clinician is the best fit.

Do you have multiple locations?
No. We're a single-clinic practice by design — it's how we keep continuity of care and same-day access reliable. Telehealth visits are available statewide for established patients.
Booking and what to expect.

How do I book?
Three ways: call +1 (415) 555-0182, use the booking form on our contact page, or — if you're an established patient — book directly through the patient portal.

Can I get a same-day appointment?
Yes. We hold back urgent slots every morning for established patients. Call before 11am and we'll fit you in that day, either in person or by telehealth.

Is telehealth available?
Yes, for established patients across California. New patients are seen in person for the first visit so we can do a proper exam and intake, then telehealth opens up for follow-ups.

What's your cancellation policy?
Cancel at least 24 hours before your appointment and there's no fee. Same-day cancellations or no-shows are billed a $50 fee — we hold time for you specifically and can't backfill it that fast.

What should I bring to my first visit?
Photo ID, your insurance card, a list of current medications and supplements, and any records from previous providers if you have them. We'll request prior records on your behalf if you'd rather.

What if I'm running late?
Call ahead. If you're more than 15 minutes late we may need to reschedule or move you to a telehealth slot — visits run long when we let them slip, and the next patient is waiting.
The money side.

Which plans do you accept?
Aetna, Anthem Blue Cross, Blue Shield of California, Cigna, UnitedHealthcare, and Medicare. If your plan isn't listed, we may still be in-network — call and we'll check.

Do I need a referral?
Not for us — family medicine is a direct-access specialty. You may need a referral from us for specialists, depending on your plan.

Do you publish cash prices?
Yes. Cash prices for the most common visits and procedures are posted on the pricing page. We quote anything custom in writing before the visit.

Do you accept HSA and FSA?
Yes — we accept HSA, FSA, and HRA cards for any eligible service. We can also provide itemized receipts if your administrator needs them.

What about out-of-network claims?
We'll provide an itemized superbill you can submit to your insurer for out-of-network reimbursement. We don't bill out-of-network insurers directly.
What we actually do.

Do you handle pediatrics?
Yes — Dr. Martin Sunak is fellowship-trained in pediatrics. Newborn-to-adolescent care, well-child visits, developmental screenings, and adolescent mental health.

Do you offer women's health services?
Yes. Dr. Amara Okafor leads women's health: annual wellness exams, contraception counseling, perimenopausal and menopausal care. We coordinate with OB-GYN specialists for pregnancy care.

Do you manage chronic disease?
Yes. Diabetes, hypertension, high cholesterol, thyroid disorders, asthma, and more. We schedule longer follow-up visits and use the portal for between-visit monitoring.

Do you give vaccinations?
Yes — full childhood and adult immunization schedules, plus travel vaccines, flu, COVID-19 boosters, and shingles. Most are covered with no copay on standard insurance.

Do you do lab work in-house?
Yes. In-house phlebotomy for common bloodwork — results often post to your portal before you leave the building. Specialized labs are sent to Quest or LabCorp.
